Notice of privacy practices.

How protected health information may be used and disclosed, and how you can get access to it.

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Who follows this notice

This notice applies to the privacy practices of Dr. Tanya Crabb, Psy.D., practicing as Soul Works by Dr. Brooklyn Chick™ (TS Crabb PLLC), and to any staff or contractors acting on behalf of the practice.

Our commitment

Protected health information, referred to here as PHI, is information that could identify you and relates to your mental or physical health, treatment, or payment for care. We are required by law to keep PHI private, to give you this notice describing our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your information without your written authorization

  • Treatment. To provide, coordinate, or manage your care, including consultation with other providers involved in your treatment.
  • Payment. To bill and collect payment for services. This practice is private pay and does not bill insurance directly. If you request a superbill for out-of-network reimbursement, the information you submit to your insurer is disclosed by you, not by us.
  • Health care operations. Quality assessment, professional review, business management, and administrative activities.
  • Required by law. When federal, state, or local law compels disclosure.
  • Serious threat to health or safety. To prevent or lessen a serious and imminent threat to you or to another identifiable person, consistent with Nevada law and professional ethical duties.
  • Abuse or neglect. Suspected abuse or neglect of a child, an older adult, or a vulnerable person, as mandated reporting law requires.
  • Judicial proceedings. In response to a court order, and in response to a subpoena only where permitted by law.
  • Health oversight and public health. To agencies authorized by law to receive such reports.
  • Business associates. To vendors that perform services on our behalf, each of which is bound by a written agreement to safeguard your information.

Uses that always require your written authorization

  • Most uses and disclosures of psychotherapy notes.
  • Any use or disclosure for marketing purposes.
  • Any sale of protected health information.
  • Most other uses not described in this notice.

You may revoke an authorization in writing at any time. Revocation does not undo disclosures already made in reliance on it.

Your rights

  • Access. Inspect and receive a copy of your record, in electronic form if you prefer, generally within 30 days of a written request.
  • Amendment. Ask that information you believe is incorrect or incomplete be amended. We may deny the request, and you may file a statement of disagreement.
  • Accounting of disclosures. Receive a list of certain disclosures made in the six years before your request.
  • Restrictions. Request limits on how information is used or shared. We are not required to agree, except that we must agree to withhold information from a health plan when you have paid for the service in full yourself.
  • Confidential communication. Ask to be contacted a particular way or at a particular address.
  • Paper copy. Receive a paper copy of this notice at any time, even if you agreed to receive it electronically.
  • Breach notification. Be notified if a breach compromises the privacy or security of your information.
  • Choose someone to act for you. A person with legal authority to make health care decisions for you may exercise these rights on your behalf.

Telehealth

Services are delivered by telehealth. Sessions are conducted on a platform that maintains encryption in transit and operates under a business associate agreement. No treatment session is conducted over website forms, ordinary email, or social media messaging.

Complaints

If you believe your privacy rights have been violated, you may complain to the practice using the contact information below. You may also file a complaint with the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/hipaa/filing-a-complaint or by calling 1-800-368-1019. You will not be retaliated against for filing a complaint.

Changes to this notice

We may change this notice and make the new terms effective for all information we maintain. The current version is always posted on this page with its effective date.
